Our Facility
Ivy Woods Manor, Guardian Elder Care, Inc. is a 91 bed licensed and certified skilled nursing home centrally located in North Lima, Ohio. Our facility sits within a wooded area and provides our residents the ability to connect with the natural surroundings.
We also provide a 20 Bed Secured Behavior Unit to meet the acute behavioral issues of residents unable to function in the general population of the facility.
Incorporated in 1995, Guardian Elder Care, Inc. brings a family environment to the nursing home industry with a commitment to excellence.
